As your Coach, I will provide unconditional support by being willing to walk along side you in whatever journey you are on without judging you, making you feel inadequate, telling you what you must do, or trying to impact the outcome. I will work with you to:

• create strategies that are tailored to meet your needs and the lifestyle you desire based on your beliefs and values
• show you how to use specific tools to help you overcome roadblocks to recovery and self-sabotaging limiting beliefs that are keeping you stuck
• create accountability so these tools and strategies will establish a firm foothold in your life and help you achieve sustainable meaningful results in whatever you choose to do
• develop a high degree of self-care and self-awareness for building and strengthening your motivation and your resolve to step into your purpose and become all you are meant to be
• Identify and initiate the links to  community-based services and resources such as housing, medical care, counseling, education, vocational training and social, spiritual or leisure activities that help you sustain and strengthen your recovery over time.  

I will be asking you many interesting and serious questions that will help raise your level of awareness about your core beliefs and habits. You are after all, the greatest expert on your life. I will help you to get in touch with that expert, your own inner coach (the one that has been hibernating as you climbed the ladder of your addiction career); that part of you that knows exactly what you want and gives you clues every day about what to do next. Working with your vision of what you want your life to look like, we will use those clues to define or redefine what recovery means to you.  Your choices will become clear, your mind will be alert, and your decisions and actions will help you experience the results you are seeking to achieve.

Coaching services are not in any way to be construed as, or a replacement for, psychotherapy, legal counsel, or medical advice. If issues arise that are best dealt with in a therapeutic context, I will refer you to one of the members of your treatment team should you have one. Otherwise, I will refer you to an appropriate healthcare professional. In signing this agreement, if you experience any difficulties during the course of this coaching process you will notify me so I may provide you with the appropriate referrals. You also agree not to hold me, or any company I am affiliated with, liable for any outcomes resulting directly or indirectly from the coaching process. 

As your coach, you can expect me to:

1. Be a partner in bringing out the best, the deepest, and the truest in you
2. Provide safety, encouragement and support; an environment in which you can relax and explore 
3. Respect the confidentiality of the agreements we make
4. Expand your view of what is possible and promote discovery of new insights
5. Give you input, straight feedback and operate as a sounding board and thinking partner
6. Listen carefully to what you say and ask questions that increase awareness
7. Be an on-going resource for you in accomplishing your intentions

I expect that you, as my client will:

1.  Cultivate honesty within yourself
2. Commit yourself to intentions that are truly meaningful and significant to you
3. Be open to my feedback, and keep me honestly informed as to what is and is not working for you
4. Take ownership for your progress and your accomplishments
5. Take financial responsibility for your coaching time. 

PRIVATE CLIENT ACCESS

Everything we will be using in this coaching experience (tools, forms, schedules, documents, resources, etc.) will all be housed conveniently on a secured and confidential "Private Client Website." Upon enrolling for the coaching experience, you will be given a unique username and password.  At your convenience you may access this "Private Client Website" area to access, review and/or update the tools, documents, resources, etc. that we’ll be using throughout our coaching.  I will provide you with the access information and further instructions upon your acceptance of this contract.

Through this Private Client Website I will provide you with a Call Strategy tool to support you in preparing for your coaching sessions so you can leverage every minute we spend together. I will also provide you with a Post Session Recap tool to support you after our coaching session is complete; this will keep the coaching going into your everyday life.  In between each coaching session you will have access to me via email and a Client Log tool so that at any time you have questions, become stuck or care to share victories I am available to you.

On occasion, you may have an urgent question, request for feedback or want a confidential response to some situation where you don't want to wait for your next call. This is  "just in time coaching" where you can use email to get a timely response. In an emergency, you may call me directly. 

 THE SYSTEM OF CONFIDENTIALITY

The Coach represents, warrants and agrees as follows:

All information provided by the Client to the Coach during the course of the coaching relationship will be deemed "Confidential Information" and kept strictly confidential unless: 

1. You give specific permission to release this information (to your health care provider for example)
2. You will be signing a specific release form to authorize this action
3. If I am legally compelled to do so. Professional privilege does not apply to a coaching relationship

Exceptions to this confidentiality agreement are when information exchanged discloses the intent to harm you, another individual, a child or an elder. 

This confidentiality restriction also applies to testimonials, which the Coach may use for promotional reasons only with the consent of the Client. The Client may request anonymity in connection with any testimonials to be used.

The Coach will return, or at the Client's request, destroy, all copies of the Confidential Information in the Coach’s possession or control upon termination of this Agreement.

DATA PRIVACY POLICY

By accepting this Agreement you acknowledge you have reviewed and accepted the terms provided in our Privacy Policy as well. Click here to open a separate window to read the Privacy Policy which includes important information such as:

* how long the data will be retained for

* your legal data protection rights

THE OPTIMAL EXPERIENCE

You may schedule your time with me on-line through the Adirondack Recovery Care website.  Once a time has been scheduled it is reserved for you. For the optimal coaching experience, schedule your sessions on a weekly basis on the same day and time. In the case that you must reschedule, I request that you kindly give 24 hours notice.

Focus sessions are 90 minutes in duration and coaching sessions are 60 minutes in duration. The E.L.I assessment and debrief includes a 20-minute on-line assessment you can take when convenient and a 60-90 minute debrief session. In between regularly appointed coaching sessions, we will use email "post-session recaps" to enhance and anchor the benefits of your coaching. This will create a "journaling" of your success. This "UnCover-E®, Living and Loving Truly Addiction-Free"  coaching program options and fee schedule are as follows:

This "UnCover-E®, Living and Loving Truly Addiction-Free" coaching program includes:

• 1 ninety-minute discovery focus session
• 5 sixty-minute regular sessions
• 1 ninety-minute family session (or regular sessions)
• 1 E.L.I assessment and 1 ninety-minute debrief focus session
• Up to ninety-minutes of bonus coaching on an as needed basis

The fee for this program is: $1347 USD.

All programs, courses and appointments will be paid using the online payment system available on the Adirondack Recovery Care website unless other arrangements have been made.

This "UnCover-E®, Living and Loving Truly Addiction-Free" coaching program has 8 sessions that will occur over the course of  2-3 months unless other arrangements are made between the coach and client.

Ninety-minutes of bonus non-expiring 1:1 coaching time will be available to you on an as needed basis after completion of the 8 program sessions.

Please Note: Bonus coaching time must be scheduled at least 24 hours in advance. Time will be used in 15-minute increments.

THE SYSTEM OF MONEY

You have two choices as to how you pay for these services:

_____PAYMENT OPTION A: Pay In Full Total: $1347 USD 

With this option, the total amount of the agreement upon acceptance of our coaching agreement. 

Payment may be made by personal check, PAYPAL, STRIPE, credit/debit card, or through the client portal via the Invoicing Menu Option, or as otherwise agreed to by coach and client. Appointments or accompaniments that are subject to additional travel, transportation or lodging requirements will be due upon invoice.

 _____PAYMENT OPTION B:  Pay per Installment (Two payments of $673.50)

With this option, the first installment would be made and automatically processed upon acceptance of this agreement. One additional installment will be automatically processed 30 days after acceptance.

Payment may be made by PAYPAL, STRIPE credit/debit card, or through the client portal via the Invoicing Menu Option, or as otherwise agreed to by coach and client. Appointments or accompaniments that are subject to additional travel, transportation or lodging requirements will be due upon invoice.

We want you to fully engage in the coaching experience and gain all of the joy and happiness of that comes from living a life free from addiction. So that you may feel comfortable with your decision, we are going to take away any financial risk you may perceive.

If after participating fully, you do not agree that the process of coaching is worth every penny of the price you paid just let us know anytime within the first 30 days of your coaching experience and we will promptly make arrangements to refund the entire amount paid.

This program is about you creating a new life, a way of being that gives you hope, fosters choice, facilitates change, maintains balance, and ultimately leads to living and loving your life truly addiction-free. If you participate fully, you'll get the kind of results outlined. If you do not show up for the calls or follow through on the commitments we’ve made together, the only guarantee is that it will not work for you!
